You can either mail them to us, or fill them out, sign them both, scan and attach them to an email.

Jewelry Party,
please be aware that we are only able to provide personal parties within a 60 miles radius of the Portland Oregon Metro Area,
and we ask that you place a $100 minimum order.
You can also receive our personal designing at a very special rate of only $20 per hour, including travel time - if applicable.
Special Custom orders take time to create especially for you,
so we usually need about 4-6 weeks for production and delivery.
